Care model designed specifically for you - Moving our of your home into a senior living facility often comes with new risks, both physical and mental. Unchecked, hospitalization rates are six times higher than seniors in community settings. And yet getting the care you need, when you need it can be especially challenging. The logistics of getting to your doctor's clinic is suddenly far more complex and most often the availability of the doctor does not align is neither timely nor convenient. Ascend addresses these by simply bringing the care to you. Our care model is purpose-built to keep long-term care patients out of the hospital; our medical providers address urgent care issues onsite in a timely fashion and perform regular (typically monthly) chronic care visits; our onsite nurses actively monitor vitals, proactively treat wounds and coordinate care with the facility and any specialty doctors that you may see.
Physical and Behavioral - Physical health conditions can significantly affect mood, cognition, and independence, while untreated mental health issues like depression, anxiety, or loneliness can lead to worsened physical outcomes and reduced quality of life. Who is eligible for Ascend Medical’s Senior Care services?
Our services are designed for residents of assisted living or senior care facilities. We are in-network with Medicare and Medicare Gap insurance, ensuring quality care with no hidden fees.
What should I expect from my experience with Ascend Senior Care?
Upon enrolling with Ascend Senior Care, the first step is our comprehensive onboarding assessment to understand your risk factors (physical, mental, metabolic and lifestyle) and work with the facility staff to design a care plan to mitigate those risks and support your healthcare goals. In addition to understanding your health needs, we look to evaluate 3 risk categories that are the most frequent causes of hospitalization:
Fall risk - by reconciling all your medications, Ascend looks for opportunities to 'deprescribe' and simplify your medication list in ways that reduce adverse reactions that often result in a loss of balance and avoidable fall risk.
Wounds/Infections - by evaluating any current wounds and lifestyle factors that can contribute to an elevated risk of wounds, Ascend builds a wound care plan that rapidly treats any existing wounds and simultaneously seeks to establish a care plan that lowers the risk of future wounds and infections.
Dementia/Psychiatric risks - moving to a new home can often be confusing and irritating. Many patients arrive with some level of dementia while others are at elevated risk of developing dementia. Ascend performs routine dementia screening every 6 months so you can identify any cognitive decline and proactively treat it.
Most long-term care patients have multiple co-morbidities, so we typically seek to conduct chronic care visits once a month with routine nursing care in between. Higher risk patients may be seen more often as needs demand.
Our nurses are also equipped with iPads and can coordinate virtual visits with doctors (including specialists) as necessary to allow our patients to avoid the hassle of leaving the facility to seek care.
Can I use Ascend Senior Care and keep my doctor?
Yes. You can keep your doctor or doctors. Regular doctors typically will not come onsite to deliver care or do so in a timely fashion. Ascend can supplement your existing doctor by providing timely acute care access as well as catering to some of your behavioral health or specialty needs. However, we will not be able to provide preventative medicine, such as annual wellness visits and care coordination - you will need to continue to visit your doctor for these. Once enrolled with Ascend, we will reach out to your doctor to educate them on our service and on how best we can work together to enhance your care.
Can you help with medication management?
Yes! Our providers help manage medications, including prescription refills and coordinating with specialists as needed.
How can I log onto telemedicine appointments?
Our telehealth links are sent 24 hour prior to your appointment. They are sent via email and SMS text message. If you are unable to retrieve your link, please contact us at 888-250-5708.
How much does Ascend Senior Care cost?
Ascend is in network with Medicare and all major Medicare Advantage plans. There are no concierge fees to Ascend and all our care is billed through your benefits (Medicare Part B). Ascend also accepts all major Medicare Supplement plans that offset the patient responsibility not covered by Medicare Part B. Patients without a Medicare Supplement plan are typically responsible for around 20% of the cost of their care.
Ascend does not take Medicaid however we do have a financial assistance program for Medicaid eligible patients that offsets any financial burden to our Medicaid patients. Please ask our Health Concierge's for more information on this program.
